The Administration for Community Living describes Area Agencies on Aging, aging and disability resource centers, and Centers for Independent Living as routes into community support. Use its directories to identify the organization serving your area.
For older adults and families, search the Eldercare Locator using your location or call 1-800-677-1116. Check its current contact options and hours on the official site. A local office can help explain which services fit the request.
Prepare a short description of the situation
Write down the state and county, the main difficulty and the outcome you want. For example: “I need help understanding bathing support and safer access to the bathroom.” Include whether the issue is urgent and whether anyone currently helps.

Ask what happens after the referral
